Who we need:

The Care Coordinator is:

  • Primarily responsible for completing phone interviews and scheduling and completing multidisciplinary team meetings for an assigned panel of recipients and their families receiving behavior analysis services to maximize the potential of the child and provide them with optimal healthcare services.
  • Also responsible for assessing, planning, implementing, and evaluating patient’s care needs.

** Preference for the selected candidate to reside and work from the state of Florida. **

What you’ll do:

  • Provide initial intake review for services to determine if all required documents have been received.
  • Complete authorization data input and workflow routing as appropriate in a timely manner.
  • Conduct phone surveys with the recipient’s guardian in a timely manner.
  • Maintains communications with Medicaid Managed Care Plans as needed.
  • Document the recipient’s assessment findings, actions, and outcomes in accordance with policy.
  • Prioritize and address requests and assignments in a professional manner to develop cooperative relationships to ensure that customer confidentiality is assured.
  • Always provide courteous and prompt service to all internal and external customers.

What you’ll need:

Required Qualifications / Experience

  • 2+ years of Pediatric experience.
  • Must have experience working in a HIPAA-compliant environment.

Preferred Qualifications / Experience

  • Medicaid experience.
  • Care coordination or case management experience.

Required Knowledge, Skills, Abilities

  • Proficient in MS Office products, including Word and Excel.
  • Effectively able to communicate within a team environment with excellent verbal and written communication skills.
